A lot of patients come to appointments expecting sensitivity, discomfort, unpleasant scraping sounds, and teeth that still don’t feel completely fresh afterwards. There is now a different approach.

EMS Guided Biofilm Therapy, or GBT, is a Swiss system designed to make professional hygiene treatment more comfortable, precise and minimally invasive — while still being highly effective.

For many patients, it completely changes the experience.

Making biofilm visible

One of the first stages of GBT is using a disclosing agent to make dental biofilm visible.

Biofilm is the soft layer of bacteria that builds up on our teeth and around the gum line. Once disclosed, the areas that need attention become clearly visible.

This is useful for you because you can actually see the areas you may be missing when brushing or cleaning between your teeth.

For us as clinicians, it means treatment is genuinely guided by where the biofilm is, rather than simply treating every surface in the same way.

AIRFLOW: removing biofilm and staining

AIRFLOW uses a controlled combination of warm water, air and a very fine erythritol powder to remove biofilm and staining.

The powder particles are only around 14 microns, making the treatment very fine and minimally abrasive.

This allows us to gently remove biofilm and common external staining, including staining from coffee and tea, without unnecessarily instrumenting the tooth surfaces.

AIRFLOW can also remove biofilm from up to 4 mm below the gum line, meaning less conventional scaling may be required in many areas.

Because external staining is removed, teeth often look and feel noticeably fresher afterwards. It can also be a useful treatment before professional teeth whitening.

Who can benefit from GBT?

GBT can be particularly useful for patients with:

  • Sensitive teeth
  • Dental implants
  • Composite bonding or veneers
  • Braces
  • Coffee or tea staining
  • Periodontal disease requiring ongoing maintenance

AIRFLOW is one of the most effective ways to maintain composite bonding because it can gently remove biofilm and external staining without relying on abrasive polishing pastes. This helps keep the composite looking clean and polished while also supporting healthier gums around the bonding — which is especially important for its long-term appearance and maintenance.

GBT is also particularly useful for implants, where gentle but thorough biofilm removal is key to keeping the surrounding gums healthy.

AIRFLOW can also be used on the tongue to remove biofilm, which can help leave the mouth feeling fresher and may help with bad breath.

Is it still a scale and polish?

Not exactly.

With traditional hygiene treatment, scaling may be used extensively before the teeth are polished.

With GBT, we first identify and remove the biofilm and staining. Any remaining harder calculus is then treated only where necessary using EMS PIEZON technology.

We treat what needs attention, without unnecessarily instrumenting every tooth surface.

What happens during a GBT appointment?

We first make the biofilm visible so both you and I can see exactly where it is. AIRFLOW is then used to gently remove biofilm and staining, and any remaining harder calculus is treated only where necessary with PIEZON.

We then show you the areas that need more attention and how to look after them between appointments.

The result is a thorough but gentle appointment, with less unnecessary instrumentation and a much clearer understanding of where to focus your home care.

What about the oral microbiome?

Our mouths naturally contain many different types of bacteria.

The aim of professional dental hygiene is not to sterilise the mouth. Instead, we want to control harmful biofilm and support a healthier oral environment.

Regular biofilm removal at home, together with professional maintenance where appropriate, helps keep this balance under control.

Gentle does not mean less effective

Patients sometimes assume that because GBT feels gentler, it must somehow be doing less.

That is not the case.

The warm water, very fine powder and targeted approach simply make treatment feel different from a traditional scale and polish while still allowing us to remove the biofilm, staining and deposits that need attention.

How to find a GBT Certified practice

If you want to find a practice that provides GBT, look for a GBT Certified practice.

These practices have completed specific EMS training and certification requirements and are listed on the official GBT Finder.

This can be particularly useful if you are travelling, moving to another city or relocating abroad and want to find a practice that follows the same GBT protocol and can continue looking after your oral health.

Why I use GBT

I like GBT because it combines prevention, comfort and precision.

We can make plaque visible, show you exactly where you are missing, remove biofilm and staining gently, treat harder deposits only where necessary and then help you improve your home care.

The goal isn’t just to make your teeth feel fresh for the day — it’s also to help you maintain healthier teeth and gums between appointments too.
